When will I get paid?
Commission checks are mailed on the 15th of each month. If you would prefer
that we deposit your commission into your checking account, please fill out the
Direct Deposit form located under the “Information” tab when you
log in to your Affiliate Account. You can either fax it or mail it in. Please
remember to include a voided blank check as well.
Please note that you must have a W-9 form on file with DentalPlans.com to
receive your commission checks. To access the form, log in to your Affiliate
Account and click on the “W-9 Form” link under the
“Information” tab. Simply print out the W-9 form and fax or mail it
to DentalPlans.com.

Will I receive a commission if a customer I
referred renews their plan?
No, you only receive a commission for the initial sale.

How can I order additional print material
like business cards and rack cards?
Log in to your Affiliate Account. At the top, you will see an “Order
Print Material” link. Here you can view all of the available print
materials and their prices. Once you decide what you would like, call Michael Sturgill at
954-668-2148 and he will order it for you. All print materials will be
customized with your unique coupon code.

Can I make my own business cards and flyers?
No, because any material representing DentalPlans.com must be legally
compliant. Therefore, we must provide all print materials to ensure that they
are in compliance. Once you join, we provide you with a free flyer, which you
can make copies of to post and pass out.

I have to pay anything to be an At-Home
Affiliate?
No, it is free to become an At-Home Affiliate. You would only need to pay for
additional print materials, if you need them.

How does a Discount Dental Plan work? Does
the customer pay yearly or monthly?
When a customer joins a discount dental plan, they will pay a one-time yearly
fee. In addition, all new members get 3 additional free months, so that’s
15 months of dental care savings! Once the customer signs up, they will receive
their membership card via email, which they can print out. If a member does not
have email, the membership card will be mailed. They can take this card to a
participating dentist to receive discounts on most dental procedures.

Can I purchase a plan for myself using my
coupon code?
Yes, you may purchase a plan for yourself and your family using your coupon
code. Once you see how this plan saves you money at the dentist, you will be
able to inform your customers with first-hand experience!

What if I am selling a plan to someone in
a different ZIP code?
You can go to http://www.dentalplans.com
and enter their ZIP code. You are welcome to print out all plan information to
present to potential customers. You can also send the customer to our web site
or give them our toll-free number. As long as they enter or mention your coupon
code, they will receive 10% off and you will receive credit for the sale.
