   • Step 1: Enter ZIP Code - A list of all the dental plans available within the selected ZIP code will appear
   • Step 2: Select the plan that's right for you or your client
   • Step 3: Join the plan - Make sure your customers use your coupon code to receive 10% off and credit for your sale.
   
   
2. Enrollment Form - Enrollment forms are located in the back pocket of your 6-page brochure.
